Enrolment Process
To enrol at Te Wairoa Medical Centre, please download the enrolment form here, or visit the clinic to collect a paper copy. Complete the form then hand it in to the clinic along with your identification documents. *
It may take up to ten business days to process your enrolment. If your matter is urgent, let our reception team know.
